| APPO Point of Service
APPO POINT OF SERVICE is a product with three benefit levels that was developed for self-insured health plans. This Plan is designed to encourage members to use the APPO regional network providers, where the greatest savings have been negotiated. It also offers savings to the Plan and in-network benefits to the members from a statewide provider network.
| APPO Level |
The highest level of benefits are paid when the member uses a regional APPO network provider. |

| In-state Network Level |
A slightly lower benefit level is paid when the member uses a provider in the statewide ppo network. |

| Out-of-Network Level |
The lowest level of benefits are paid when the member uses a provider that is neither a regional APPO network provider nor a statewide ppo network provider. |
