A traditional PPO product for self- funded and fully- insured groups.  Members may use in or out- of- network providers. Benefits are greater when the member uses network providers.

A product with three benefit levels for self- funded plans.

APPO MEDICAL BUDGET
A plan in which APPO providers share claim risks with a self- insured employer. Members agree to use APPO providers exclusively.
